Our Diabetic Care Management program provides an interdisciplinary approach to the challenges of managing diabetic care and complications. Every resident with diabetes receives an individualized assessment of their blood sugar control with lab testing and glucose monitoring. The Diabetes management team then reviews the results of the labs and monitoring.
Our Diabetic Care team provides a comprehensive, individualized approach to our resident’s care plan. They consist of:
- Physician
- Medical Director
- Director of Nursing
- Registered Dietitian
- Pharmacist
- Licensed Nursing Staff
- Rehabilitation Staff
We also partner with residents and their families to teach a nationally recognized curriculum on diabetic care and self-management skills. We work with them until they feel l knowledgeable and comfortable transitioning home or to a lower level of care.
